The purpose of this study is to develop and test a stroke risk management program intended to help patients improve control of the health conditions that increase their risk of stroke compared to standard care.

The purpose of this study is to develop and test a stroke risk management program intended to help patients improve control of the health conditions that increase their risk of stroke compared to standard care.

Eligibility of study

You may be eligible for this study if you meet the following criteria:

  • Conditions: Stroke
  • Age: 18 years - 100 years
  • Gender: All

This study investigates a stroke risk management program to see if it can help patients better control the conditions that increase their risk of stroke compared to standard care. Stroke is a medical condition where poor blood flow to the brain results in cell death, and it is important to manage risk factors to prevent it.

Participants in this study will be assigned to different study arms. One arm will follow the stroke risk management program, while the other will receive standard care. The study will monitor and compare how well participants in each arm manage their stroke risk factors.

  • Who can participate: The study is open to adults aged 18 and older who have been identified as having risk factors for stroke. Key eligibility criteria include a history of hypertension or diabetes.
  • Study details: Participants will be assigned to either follow a stroke risk management program or receive standard care. Their progress in controlling stroke risk factors will be monitored and compared.
