The overall goal is to compare the effectiveness of the current implementation of Cerner electronic medication reconciliation in increasing discharge medication compliance in line with NHIQM specifications for patients with ischemic stroke and acute myocardial infarction.

The overall goal is to compare the effectiveness of the current implementation of Cerner electronic medication reconciliation in increasing discharge medication compliance in line with NHIQM specifications for patients with ischemic stroke and acute myocardial infarction.

Eligibility of study

You may be eligible for this study if you meet the following criteria:

  • Conditions: Stroke,Heart Attack
  • Age: 100 years or below
  • Gender: All

This study investigates how well the current electronic medication reconciliation system works for patients who have had an ischemic stroke or acute myocardial infarction, which is another term for a heart attack. Medication reconciliation is a process that ensures patients are given the correct medications when they leave the hospital.

Participants in the study will be monitored to see how well they follow their medication plans after being discharged from the hospital. This will help determine if the electronic system improves medication compliance according to set healthcare standards.

  • Who can participate: Adults aged 18 and older who have experienced an ischemic stroke or acute myocardial infarction, and are able to provide informed consent, are eligible to participate. Additional criteria include the ability to adhere to prescribed medication plans.
  • Study details: The study will observe if the electronic system maintains medication compliance. The investigational system's effectiveness will be assessed by monitoring participants post-discharge.
