The purpose of this study is to determine whether a computer model for capturing data to fill form CMS-2728 "End Stage Renal Disease Medical Evidence Report" improves prediction of mortality over providers completion of the form.

The purpose of this study is to determine whether a computer model for capturing data to fill form CMS-2728 "End Stage Renal Disease Medical Evidence Report" improves prediction of mortality over providers completion of the form.

Eligibility of study

You may be eligible for this study if you meet the following criteria:

  • Conditions: Healthy
  • Age: 100 years or below
  • Gender: All

This study investigates whether using a computer model to fill out the CMS-2728 form, which is the End Stage Renal Disease Medical Evidence Report, can better predict mortality compared to when healthcare providers complete the form themselves. End Stage Renal Disease is the last stage of chronic kidney disease, where the kidneys can no longer support the body's needs.

Participants in the study will have their data processed through a computer model designed to complete the CMS-2728 form. The study will compare the model's predictions of mortality with those made by healthcare providers to evaluate the model's effectiveness.
