The purpose of this study is to evaluate the efficacy of 6-month duration of the Trauma Medical Home collaborative care intervention in improving the functional and psychological recovery of injury survivors, enhancing their quality of life and reducing their acute health care utilization.

This study investigates the Trauma Medical Home collaborative care intervention for individuals who have experienced injuries. The goal is to see how well this intervention helps improve the functional and psychological recovery of injury survivors, which means helping them get back to their daily activities and feel better emotionally. The study also looks at whether this intervention can enhance their quality of life and reduce the need for urgent health care visits.
Participants in the study will receive the Trauma Medical Home collaborative care intervention. This involves a team-based approach to healthcare, where different healthcare providers work together to support the patient's recovery. The study will measure how this intervention affects participants' recovery and healthcare use over a six-month period.
