Brief description of study.

The purpose of this study is to reduce patients who have had a hip and or knee replacement re-encounters to either the emergency department or re admission to the hospital after their discharge from the hospital within 90 days of discharge.

The purpose of this study is to reduce patients who have had a hip and or knee replacement re-encounters to either the emergency department or re admission to the hospital after their discharge from the hospital within 90 days of discharge.

Eligibility of study

You may be eligible for this study if you meet the following criteria:

  • Conditions: hip replacement,knee replacement
  • Age: 100 years or below
  • Gender: All
