We propose a descriptive, retrospective study involving medical records review of GRACE patients at IU Health, with the objective of identifying the medication interventions recommended by GRACE team care and adoption of recommendations.

We propose a descriptive, retrospective study involving medical records review of GRACE patients at IU Health, with the objective of identifying the medication interventions recommended by GRACE team care and adoption of recommendations.

Eligibility of study

You may be eligible for this study if you meet the following criteria:

  • Conditions: Healthy
  • Age: 100 years or below
  • Gender: All

This study investigates the medication interventions recommended for GRACE patients. GRACE stands for Geriatric Resources for Assessment and Care of Elders, a program designed to improve the quality of care for older adults.
